Nursing Care Services

Our nursing services are performed by registered nurses (RNs) and licensed vocational nurses (LVNs) who are trained to identify safety concerns in the home environment.

For example, one of best ways to keep elderly patients safe at home is to be proactive about preventing falls.

To do that, our nurses survey the home and look for potential fall hazards and then work to address these situations. A recommendation to install an extra handrail in a bathroom is much better than a trip to the emergency department.

Skilled nursing services can include wound care, injections, tube feedings, disease management, medication management, catheter changes, observation and assessment of the patient’s condition, management and evaluation of the care plan, and patient/caregiver education.

Our nurses:

  • Monitor a patient's vital signs, blood pressure and heart rate

  • Educate patients about medications, and any special dietary needs

  • Control the impact of acute and chronic illnesses such as diabetes and heart disease

  • Help reduce the complications caused by wounds and ostomies

  • Teach self-care techniques for a multitude of issues

  • Arrange for additional services, if needed, and communicate with others across the home health care team

  • Coordinate a plan of care with the patient’s medical team.
