MemorialCare Home Health caregivers will bring quality care right to the patient’s doorstep, and oftentimes, home health care services are available at no out-of-pocket cost to the patient!

We’ll be your advocate every step of the way to make this process easy and stress-free. We’ll do the legwork to establish care, will work with insurance companies and will find every funding source possible, reducing - and most often eliminating - any financial burden for the patient. We’ll make the transition to home health care easy and stress-free so you can focus on what matters most: Healing and recovery.

Who pays for home health services?

MemorialCare Home Health is a certified provider for Medicare, Medicaid and many private insurance companies. Oftentimes, home health services are covered at 100% for patients in need – meaning there is no out-of-pocket cost.

All patients that receive home healthcare services under Medicare regulations must be homebound and must be under the care of a primary care provider. Home care patients must require the skill of a nurse or a physical or speech therapist for services to be initiated and covered under Medicare. If covered, Medicare pays 100% of home care services.

We know insurance can be complex, so we’ll do all the legwork for you. Just give us a call and we’ll help navigate insurance coverage to eliminate or minimize any financial burden. Oftentimes, we’re able to find additional funding sources to eliminate all out-of-pocket costs for the patient.

How do I get started with MemorialCare Home Health services?

It’s important to know that every patient can choose their preferred home health service provider. It’s as simple as making a phone call to our office, and then we’ll navigate the process for the patient. Patients can inquire with their primary care provider about the benefits of home health care at any time. Some providers and organizations have partnerships with home health service providers, but the patient is ultimately in control of selecting the best provider for his/her needs. There are two ways a patient can start services with MemorialCare Home Health:

Establishing Care for Yourself

Patients can inquire about home health services for him/herself by either calling MemorialCare Home Health directly or talking with his/her primary care provider.

If calling us directly, we can talk through any challenges and needs and help the potential patient determine what home health services would be beneficial. Then, we can contact the primary care provider to write an order for services. At that point, we’ll do the rest.

If talking with the primary care provider directly about home health care services, the patient can request for the primary care provider to write an order for a registered nurse to evaluate the patient for home health services. The patient should request the order to be faxed or called in to MemorialCare Home Health at:

At this point, we’ll do the rest. Once we get the order, we will call the patient to let him/her know we have received an order for a nurse to come out and we will arrange the first visit. After the first visit, we communicate our findings with the primary physician, and together, with the patient’s involvement, we develop a Plan of Care. Our goal is to provide the best and most compassionate care that promotes health, healing and independence.

Establishing Care for a Family Member or Friend

Family members and friends who are concerned about a loved one taking medications correctly, being home alone or changing in condition can initiate a home health referral by calling MemorialCare Home Health directly. We’ll do the legwork from here, including working with the primary care provider to obtain an order for home health care services.

At that point, the physician will either want to see the patient for an evaluation or will write an order for a registered nurse to evaluate the patient for home health care services.  

If contacted directly, those seeking to help should ask the primary care provider to call in or fax the order to MemorialCare Home Health:

MemorialCare Home Health will call the patient to let him/her know we have received an order for a nurse to come out to see them and we will arrange the first visit.
