Medical Social Services
Our medical social workers help patients and their caregivers navigate the complexity that often comes with managing home health care.
Our team members will help patients and their families determine needs, guide the patient to beneficial community resources and support, provide education for the patient and caregivers, and help with long-term planning upon discharge from our home health services.
Through our medical social services, our team will help:
Assess a patient’s social, economic and emotional needs relating to the overall medical plan.
Provide education and access to community resources for patients and their caregivers.
Assess any additional solutions that may be needed, such as additional home health or community services.
