Case Management

Our registered nurses are tasked to oversee the care of their patients.

This means they are constantly checking in to make sure the care we are providing is meeting the needs of the patient and the goals of the patient are being met.

They are experts in home health care and identify needs for other disciplines, additional services, and communicate with internal team members and physicians on an ongoing basis.
