Care. Compassion.
CARING.

We’ll help you achieve health, healing and independence.

At MemorialCare Home Health, our priorities are two-fold: To provide the highest-quality, patient-centered care possible and to provide the most compassionate caring. Together, these create our unique approach to providing care - and caring - for our patients and their caregivers.

We know transitioning to home health care can seem challenging, and we often find that patients and caregivers aren’t sure whether home health services or an extended care facility provide the best solution for recovery. We’ll advocate every step of the way to keep patients in their homes and find every funding source possible, reducing - and most often eliminating - any financial burden for the patient.

We’ll make the transition easy and stress-free so patients and caregivers can focus on what matters most: Healing and recovery.

We also know that medical professionals coming into a patient’s home is very personal, so we carefully select caregivers who not only exceed our highest quality care standards, but who also serve every patient with a compassionate, caring approach to home health care.

This compassionate approach only enhances our high quality of care standards – in fact, our Medicare.gov quality ratings far exceed both state and national averages. As part of the MemorialCare health system, you can rest assured knowing our home care standards are consistent with the excellent care you expect from MemorialCare.

Home Health Services

  • Nursing Care Services

Our registered nurses provide direct care; manage, observe and evaluate a patient’s care; and teach the patient and his/her caregiver. The RN, oftentimes along with the licensed vocational nurse (LVN), will change dressings, draw blood for lab tests, monitor vital signs, and oversee the overall plan of care.

  • Physical Therapy Services

Our physical therapists treat injury and disease by mechanical means, including manual, passive and active range of motion, exercise and modalities that enhance and engage patients to increase their functionality and strength.

  • Occupational Therapy Services

Our occupational therapists help patients return to activities of daily living (like bathing, preparing meals and housekeeping) after illness or injury. They often provide support for injuries involving the hand and shoulder, as well as fine motor skill rehabilitation.

  • Speech Therapy Services

Our speech therapists assist with problems involving speech, language and swallowing. Communication problems can develop after an injury or illness, like a stroke or an accident. They can also assist with cognitive issues and assist patients with dementia and Alzheimer’s.

  • Medical Social Services

Our medical social workers help patients and families with social and emotional concerns related to illness, including counseling or help in finding community resources. They can often help sort through the financial and benefit complexities that so many elderly face.

  • Home Health Aide Services

Our Aides help patients regain independence by lending support with activities of daily living, such as eating, bathing, getting dressed and toileting. They can also help prepare meals and perform light housekeeping duties while the patient is recovering from an illness.

  • Case Management

Our registered nurses are tasked to oversee the care of their patients. They routinely check in to make sure the care we are providing is meeting the needs of the patient and the goals of the patient are being met. They identify needs for other disciplines, additional services, and communicate with internal team members and physicians on an ongoing basis.

Benefits of Home Health Care

Research shows that when it comes to physical and mental well-being, there is no place like home.

>> Receiving medical care in the home gives patients a sense of control, satisfaction and independence, and evidence shows that we heal more quickly at home.

>> Home health care keeps families together for support and socialization, allowing individuals and their families to take an active role in care and recovery.

>> Home health care helps prevent or postpone hospitalization or the need for institutional care, and costs less than inpatient care.
